Crohn's CDAI Score: Why Hematocrit and Symptoms Both Count

The Crohn’s Disease Activity Index, usually shortened to CDAI, is a weighted score developed for adult Crohn’s research. It combines a seven-day symptom diary with examination findings, medication use, body weight, and hematocrit. That mix explains why a CDAI result may seem disconnected from a single day’s symptoms. It also explains a common surprise: hematocrit is part of the formula, but CRP and fecal calprotectin are not. CDAI remains a familiar clinical-trial endpoint, yet modern consensus work pairs patient-reported outcomes with biomarkers and endoscopy because no clinical index fully represents inflammation (original CDAI paper; CORE-IBD consensus).

What goes into a CDAI score

The calculation uses eight items. Three come from a seven-day diary: the number of liquid or very soft stools, abdominal-pain ratings, and general well-being ratings. The remaining inputs are complications, use of antidiarrheal medication, an abdominal-mass assessment, hematocrit, and percentage difference from a reference body weight. Each input is multiplied by a coefficient before the pieces are added. That weighting is why you cannot reproduce CDAI accurately by simply counting how many problems are present (development and validation review).

Commonly used bands describe a score below 150 as remission, 150 to 219 as mild activity, 220 to 450 as moderate-to-severe activity, and above 450 as severe activity. These labels are research conventions, not stand-alone diagnoses. Trial definitions may also require a minimum fall in CDAI, often called a response, rather than only asking whether someone crossed one cutoff. Always check the protocol or note that supplied the number.

Complications in the formula can include joint symptoms, eye or skin manifestations, fever, fistula-related problems, and other specified findings. A clinician determines some inputs, while the diary depends on consistent recording. Missing days, changing the meaning of “liquid stool,” or estimating a week from memory can change the total. If a score matters for trial eligibility or a treatment decision, confirm how the diary and body-weight reference were handled.

Why hematocrit counts but other labs do not

Hematocrit is the fraction of blood volume made up by red blood cells. In CDAI, the calculation penalizes a hematocrit below a sex-specific reference value. This makes anemia-related information part of the total, but hematocrit is not a direct measurement of bowel inflammation. It can be affected by blood loss, iron deficiency, vitamin deficiency, hydration, and other conditions. A change in hematocrit can therefore shift CDAI even when the bowel’s inflammatory appearance has not changed.

CRP, ESR, albumin, hemoglobin as a separate value, and fecal calprotectin do not enter the original equation. They may still be ordered because they answer different questions. AGA guidance recommends using biomarkers together with symptoms in Crohn’s management, and identifies settings in which fecal calprotectin and CRP can help assess whether further endoscopic evaluation is needed (AGA biomarker guideline; AGA clinical guidance). A CDAI calculator that asks for CRP is therefore using an adaptation or an additional field, not the original eight-item formula.

CDAI does not equal intestinal healing

CDAI can rise because of diarrhea, pain, poor well-being, anemia, or complications that may have several causes. It can also be low while inflammation remains visible. A prospective comparison found that CDAI and HBI were not reliable substitutes for endoscopic assessment of remission, while CRP and calprotectin supplied complementary objective information (clinical scores and endoscopic remission). Treat-to-target recommendations similarly separate clinical remission, biomarker improvement, and endoscopic healing (STRIDE-II).

For a useful conversation, keep the seven diary days, the hematocrit value and lab date, current weight, medication changes, and any recent calprotectin, CRP, imaging, or endoscopy results together. Do not adjust treatment from a self-calculated score. Contact your care team promptly for severe or rapidly worsening pain, persistent vomiting, heavy bleeding, fever, fainting, or dehydration. CDAI is best treated as a structured summary of clinical activity, not a verdict about what the bowel looks like. When comparing two values, confirm that both used complete seven-day diaries, the same reference-weight method, and appropriately timed hematocrit results. Ask which component drove the difference and whether the change met the response definition used by that clinic or study. Keeping the component worksheet prevents a modest anemia change or one unusual week of stools from being mistaken for a complete account of treatment response.
