Crohn's CDEIS Score: What It Measures on Colonoscopy

The Crohn’s Disease Endoscopic Index of Severity, or CDEIS, turns colonoscopy findings into a numerical measure of visible Crohn’s activity. It was designed mainly for research and is more complicated than the SES-CD score now used in many trials. CDEIS does not include pain, bowel-movement frequency, CRP, fecal calprotectin, or biopsy results. It grades lesions and narrowing in bowel segments reached during ileocolonoscopy. That makes it an objective mucosal score, but not a complete picture of transmural Crohn’s disease or symptoms (Crohn’s endoscopic-index review; endoscopic activity review).
What the endoscopist records
CDEIS divides the examined bowel into five segments: the rectum, sigmoid and left colon, transverse colon, right colon, and ileum. In each segment, the reader notes whether deep ulcers or superficial ulcers are present and estimates the proportion of surface that is ulcerated and the proportion affected by any Crohn’s lesion. The calculation also accounts for ulcerated and non-ulcerated stenosis, or narrowing. A formula combines those observations and adjusts for the number of segments examined (endoscopic disease-activity review).
The underlying description matters. Deep ulceration, widespread affected surface, and stenosis communicate different clinical information even if two totals happen to be similar. A segment that could not be reached can also affect interpretation. Save the procedure report, bowel-preparation quality, extent reached, and whether biopsies were taken along with the score.
CDEIS is not a biopsy index. Microscopic inflammation is graded with a histology instrument, and structural complications beyond the bowel lining may require cross-sectional imaging. Crohn’s can involve the full bowel wall or small-bowel areas that the colonoscope does not reach, which is one reason endoscopy is combined with imaging in selected patients (MRE-index review).
CDEIS versus SES-CD
SES-CD was developed as a simpler endoscopic score. It grades ulcer size, ulcerated surface, affected surface, and narrowing in five segments. CDEIS distinguishes superficial from deep ulcers and uses estimated continuous surface percentages, making its calculation more laborious. Both measure luminal Crohn’s activity, and neither includes routine laboratory values (endoscopic indices review).
Numbers from the two scales cannot be substituted directly. Terms such as endoscopic response, remission, or healing also depend on the study’s definition. A trial may define response as a percentage reduction from baseline, while another uses an absolute threshold. When reading a result, ask which index and cutoff were used rather than translating it through a generic online table.
Where labs and symptoms fit
CRP and fecal calprotectin do not change the CDEIS calculation, but they can help place it in context. Symptoms may improve without complete ulcer healing, and symptoms can persist for reasons other than active mucosal inflammation. Treat-to-target guidance therefore separates clinical remission, biomarker improvement, and endoscopic healing (STRIDE-II). Consensus trial outcomes also report patient-reported symptoms, CRP, calprotectin, and SES-CD as distinct domains rather than blending them into one score (CORE-IBD).
For longitudinal tracking, keep the CDEIS total beside the segment-level findings, photographs if available, current treatment, symptoms, CRP, calprotectin, and imaging. Comparisons are strongest when procedures use similar technique and the same score. Do not interpret a small numerical difference without the endoscopist’s assessment of whether it is meaningful. CDEIS answers a focused question: how much visible Crohn’s injury was present in the examined mucosa on that date. It does not explain every symptom or rule out disease beyond the scope’s reach.
Questions that make the number more useful
Before comparing this result with an older one, confirm that both records used the same named instrument, version, scoring range, and assessment window. Ask whether the total represents the worst segment, a sum across segments, an average, or a single day. If a clinician or study used a modified version, record exactly what was removed or added. Also check whether treatment, bowel preparation, infection, missed diary entries, or incomplete examination affected the inputs. A small apparent change may reflect measurement conditions rather than a meaningful change in disease.
At a visit, useful questions include: What domain does this score measure? Which component drove the result? What would count as response or remission in this setting? Does the number need confirmation with blood work, stool testing, endoscopy, biopsy, or imaging? Is the goal to follow symptoms, current inflammation, structural damage, treatment response, or quality of life? Asking these questions turns an unfamiliar total into a specific clinical conversation.
For personal tracking, avoid combining unlike scores on one graph. Keep each instrument in its own series and place objective tests on parallel timelines. Save the source report because the component details may matter later. Note both the sample or procedure date and the date you received the result. Trends are easiest to interpret when the method and timing stay consistent. If the number conflicts with how you feel or with another test, bring the disagreement to your care team instead of choosing the result that seems most reassuring. Discordance is often the reason to look more closely.
