Crohn's Harvey-Bradshaw Index: What Your HBI Means

The Harvey-Bradshaw Index, or HBI, is a short clinical score used to summarize Crohn’s disease symptoms and complications. It is easier to complete than the CDAI and appears often in clinic notes, registries, and pragmatic trials. HBI is useful shorthand, but it is not a blood test and does not directly measure ulceration. Its five parts are general well-being, abdominal pain, liquid stools, abdominal mass, and complications. CRP, fecal calprotectin, CBC, and albumin are not included (original HBI report; IBD activity-index review).
How the HBI is calculated
General well-being and abdominal pain are graded on short severity scales. Liquid stools are counted for the previous day. A clinician grades whether an abdominal mass is absent, questionable, definite, or definite and tender. Specified complications add points individually; examples include joint pain, eye inflammation, skin manifestations, anal fissure, fistula, abscess, and fever. Because each complication can add to the total, two people with the same stool and pain pattern may receive different scores.
Common interpretation uses 4 or less for remission, 5 to 7 for mild activity, 8 to 16 for moderate activity, and above 16 for severe activity. Cutoffs can vary by study, so the definition attached to a particular result matters. The HBI also depends on how accurately the prior day represents the patient’s pattern. A one-day count after bowel preparation, travel, infection, or unusual eating may not reflect a typical week.
HBI is related to CDAI but is not a shortened copy that produces the same number. CDAI uses a seven-day diary, weighted coefficients, weight, hematocrit, antidiarrheal use, and additional clinical inputs. A 2025 analysis found strong correlations between HBI and Crohn’s PRO-2 in trial data, but the prediction ranges for individual patients were wide, illustrating why conversion estimates should not replace the recorded instrument (HBI and PRO-2 analysis).
Why lab tests sit beside the HBI
No routine laboratory value is part of HBI. A low score therefore means the selected symptoms and findings are limited; it does not prove that CRP or calprotectin is normal, and it does not prove mucosal healing. Conversely, a high HBI does not prove active bowel inflammation. Diarrhea and pain may also arise from infection, bile-acid diarrhea, stricturing, medication effects, or overlapping functional symptoms.
This gap is why contemporary monitoring combines patient experience with objective measures. A study comparing clinical indices with endoscopy found that HBI and CDAI could not reliably identify endoscopic remission by themselves (clinical indices versus endoscopy). AGA guidance recommends combining symptoms with biomarkers and using endoscopic assessment when the combination leaves important uncertainty (AGA Crohn’s biomarker guidance). STRIDE-II likewise treats symptoms, biomarkers, and endoscopic healing as distinct targets over different time horizons (STRIDE-II).
Making your HBI more useful
If HBI appears in your record, note the date and each component rather than saving only the total. Record the previous day’s liquid-stool count, pain, general well-being, complications, and any reason that day was unusual. Add nearby CRP, calprotectin, hemoglobin, albumin, medication changes, imaging, and scope findings. That context helps your care team see whether symptoms and inflammation are moving together.
Do not use HBI bands to diagnose a flare or change medication on your own. A rapid increase is information to share, particularly if accompanied by fever, vomiting, escalating pain, abdominal swelling, dehydration, or substantial bleeding. HBI works best as a repeatable clinic snapshot. The deeper question is not simply whether the total is “good,” but whether the symptoms, biomarkers, and bowel assessment together support the same conclusion.
Questions that make the number more useful
Before comparing this result with an older one, confirm that both records used the same named instrument, version, scoring range, and assessment window. Ask whether the total represents the worst segment, a sum across segments, an average, or a single day. If a clinician or study used a modified version, record exactly what was removed or added. Also check whether treatment, bowel preparation, infection, missed diary entries, or incomplete examination affected the inputs. A small apparent change may reflect measurement conditions rather than a meaningful change in disease.
At a visit, useful questions include: What domain does this score measure? Which component drove the result? What would count as response or remission in this setting? Does the number need confirmation with blood work, stool testing, endoscopy, biopsy, or imaging? Is the goal to follow symptoms, current inflammation, structural damage, treatment response, or quality of life? Asking these questions turns an unfamiliar total into a specific clinical conversation.
For personal tracking, avoid combining unlike scores on one graph. Keep each instrument in its own series and place objective tests on parallel timelines. Save the source report because the component details may matter later. Note both the sample or procedure date and the date you received the result. Trends are easiest to interpret when the method and timing stay consistent. If the number conflicts with how you feel or with another test, bring the disagreement to your care team instead of choosing the result that seems most reassuring. Discordance is often the reason to look more closely.
