Geboes Score for Ulcerative Colitis Biopsies Explained

The Geboes score is a detailed microscopic grading system for ulcerative colitis biopsies. It organizes changes into hierarchical categories, moving from structural and chronic inflammatory changes through neutrophilic activity, crypt destruction, and erosion or ulceration. It does not use symptoms, endoscopic appearance, CRP, or fecal calprotectin. The result describes what a pathologist sees in sampled tissue, and the score or cutoff must be reported precisely because papers use several Geboes formats and definitions (original Geboes scale; ECCO histology overview).
The Geboes grades in plain language
Grade 0 addresses architectural changes, such as distorted crypt structure. Grade 1 grades chronic inflammatory infiltrate. Grade 2 records neutrophils or eosinophils in the lamina propria. Grade 3 captures neutrophils in the epithelium, including cryptitis. Grade 4 represents crypt destruction, and grade 5 represents erosion or ulceration. Subgrades add detail within the categories (original Geboes scale).
This hierarchy distinguishes chronic evidence of UC from active neutrophilic inflammation. A report may describe chronic colitis while stating that no active inflammation is present. That is not contradictory: longstanding architectural change and acute activity are different microscopic features.
Researchers may report the original category and subgrade, a converted continuous value, or a threshold such as less than 2.0 or less than 3.1 for a particular definition of histologic remission. Those thresholds do not mean the same thing. Always capture the exact notation and study definition rather than saving only “Geboes negative.”
How Geboes compares with newer indices
The Nancy Histological Index compresses activity into five grades. The Robarts Histopathology Index uses a weighted combination of chronic inflammatory infiltrate, lamina propria neutrophils, epithelial neutrophils, and erosion or ulceration. Geboes is broader and more granular, but that detail can make consistency and endpoint interpretation harder. Contemporary consensus recommendations prefer clearly validated indices and explicit definitions of activity and remission (histopathology consensus). A Geboes number should not be converted into Nancy or RHI using a casual lookup table.
Why labs and scope findings can differ
CRP reflects systemic inflammation and may stay normal in some people with active UC. Calprotectin reflects intestinal neutrophilic inflammation but samples stool, while Geboes grades tiny tissue samples from named locations. MES or UCEIS records visible endoscopic activity. Timing, treatment, sampling distribution, and biological variability can produce disagreement.
STRIDE-II recognizes histology as an adjunctive measure of deeper UC remission rather than a stand-alone formal target (STRIDE-II). Save the exact Geboes format, highest grade, biopsy locations, pathology narrative, date, endoscopic score, medications, symptoms, CRP, and calprotectin. Ask whether the result represents the worst segment or another summary. The score is most informative when it remains attached to the tissue and definition that produced it.
Questions that make the number more useful
Before comparing this result with an older one, confirm that both records used the same named instrument, version, scoring range, and assessment window. Ask whether the total represents the worst segment, a sum across segments, an average, or a single day. If a clinician or study used a modified version, record exactly what was removed or added. Also check whether treatment, bowel preparation, infection, missed diary entries, or incomplete examination affected the inputs. A small apparent change may reflect measurement conditions rather than a meaningful change in disease.
At a visit, useful questions include: What domain does this score measure? Which component drove the result? What would count as response or remission in this setting? Does the number need confirmation with blood work, stool testing, endoscopy, biopsy, or imaging? Is the goal to follow symptoms, current inflammation, structural damage, treatment response, or quality of life? Asking these questions turns an unfamiliar total into a specific clinical conversation.
For personal tracking, avoid combining unlike scores on one graph. Keep each instrument in its own series and place objective tests on parallel timelines. Save the source report because the component details may matter later. Note both the sample or procedure date and the date you received the result. Trends are easiest to interpret when the method and timing stay consistent. If the number conflicts with how you feel or with another test, bring the disagreement to your care team instead of choosing the result that seems most reassuring. Discordance is often the reason to look more closely.
