Getting a high fecal calprotectin result followed by a colonoscopy that looks completely normal is one of the more confusing situations in a gastrointestinal workup. The stool test points toward inflammation somewhere in the digestive tract, yet the camera that examined your colon found nothing wrong. If you are trying to make sense of a high calprotectin with a normal colonoscopy, the short version is that the two tests measure different things and cover different territory. Understanding why they can disagree, and knowing which questions to raise at your next appointment, turns a frustrating gray zone into a clear set of next steps.

What a high calprotectin actually measures

Calprotectin is a protein found mainly in neutrophils, the white blood cells that flood into tissue during inflammation, where it makes up a large share of the cell's cytoplasmic protein and is released when those cells are activated or damaged, according to a review in the Journal of Laboratory and Precision Medicine. When the intestinal lining is inflamed, neutrophils migrate into the gut wall and shed calprotectin into the stool, which is why the Crohn's & Colitis Foundation describes the fecal calprotectin test as a stool test for intestinal inflammation that helps predict active disease. The test is sensitive, with a pooled sensitivity around 93% and specificity around 96% for detecting mucosal inflammation in adults. A result under roughly 50 µg/g is generally treated as normal, and the American Gastroenterological Association suggests a threshold near 150 µg/g to help rule active Crohn's inflammation in or out. A high number signals that inflammation is likely present, but it does not say where.

Why a colonoscopy can look normal

A colonoscopy is thorough within its reach, and that reach is limited. It lets the doctor examine the rectum, the colon, and the very end of the small intestine, called the terminal ileum, as NIDDK explains. It cannot see most of the small bowel, which runs for many feet between the stomach and the terminal ileum. Crohn's disease frequently involves the small intestine, and the Crohn's & Colitis Foundation notes that portions of it cannot be easily seen by colonoscopy or endoscopy. A scope that reaches a healthy-looking colon and terminal ileum can therefore still miss active disease higher up. This is the most common reason for elevated calprotectin but colonoscopy clear results: the inflammation sits in a segment the colonoscope never reached, so small bowel Crohn's is missed on colonoscopy.

Evaluating the small bowel

When calprotectin stays high and the colon looks normal, the small bowel is the natural next place to look, and several tools reach where a standard scope cannot. Capsule endoscopy uses a pill-sized camera that you swallow to photograph the small intestine as it travels through, one of the tests NIDDK lists for checking the small bowel. Cross-sectional imaging such as CT or MR enterography uses oral contrast to show the bowel wall. In a comparison published in the Journal of the Canadian Association of Gastroenterology, capsule endoscopy detected active inflammation throughout the small bowel far more often than MR enterography and identified disease at sites proximal to the area a colonoscopy examines. Enteroscopy, a longer scope, can reach deeper segments directly. Which test fits depends on your symptoms and whether intestinal narrowing is a concern.

Non-IBD reasons calprotectin can be high

Small bowel Crohn's is not the only explanation for a high result. Calprotectin rises with many kinds of gut inflammation. Nonsteroidal anti-inflammatory drugs, or NSAIDs, can cause an enteropathy that raises levels, and gastrointestinal infections including bacterial and viral enteritis and Clostridioides difficile can push them up as well, per the Journal of Laboratory and Precision Medicine review. That same review notes that colorectal cancer and microscopic colitis have also been linked to higher readings, though calprotectin lacks the specificity to diagnose any of these on its own. Timing matters too, since one elevated value can reflect a transient problem. Repeating the test and reviewing recent medications and infections with your doctor helps separate a real signal from background noise before committing to more invasive testing.

Questions to ask your GI about conflicting test results

Walking into your appointment with organized questions makes the visit more productive, especially when your goal is reconciling conflicting test results. Consider asking:

  • Did my colonoscopy reach and biopsy the terminal ileum, and could inflammation be sitting higher in the small bowel where the scope could not see?
  • Should I have small bowel imaging or capsule endoscopy to evaluate the areas the colonoscopy missed?
  • Could NSAIDs, a recent infection, or another factor explain the result, and should we repeat the calprotectin test before doing more?

Bring the actual numbers, the dates of each test, and a note of any symptoms. The AGA emphasizes combining biomarkers with clinical judgment rather than reading either result in isolation, and the ACG similarly recommends using fecal calprotectin alongside symptoms and endoscopic findings to assess disease activity.

Making sense of the mismatch

A high calprotectin with a normal colonoscopy is a common and usually explainable mismatch rather than a contradiction. The stool test flags inflammation without pinpointing it, and the colonoscopy sees only part of the digestive tract. The gap between them often points toward the small bowel or a non-IBD cause worth ruling out, which is why the next steps for a high calprotectin are more testing and context rather than a single definitive answer. Keeping your results, their dates, and your unanswered questions in one place lets you and your gastroenterologist work through the findings methodically and choose the right follow-up test instead of guessing.
