How to Tell Whether Your Biologic Is Working: Symptoms, Calprotectin, and Scope Results

Starting a biologic for Crohn's disease or ulcerative colitis raises an immediate question that drug information pages rarely answer: how do you know if it is actually working? Feeling better is encouraging, but feeling better and healing are separate things, and inflammatory bowel disease is unusually good at hiding damage behind quiet symptoms. Gastroenterologists judge biologic response using three signals read together, and each of them tells you something the other two cannot.
What "working" actually means in IBD care
Modern IBD practice defines treatment success in stages rather than as a single yes or no. The STRIDE-II consensus from the International Organization for the Study of IBD sets symptomatic relief and normalization of serum and fecal markers as short-term targets, with clinical remission, endoscopic healing, restored quality of life, and absence of disability as long-term targets. That framing matters when you are trying to interpret your own progress. A biologic that cuts your urgency in the first month is meeting an early target. Whether it is meeting the deeper target of healing the bowel lining is a question that symptoms alone cannot answer, which is why your gastroenterologist keeps ordering stool tests and scopes even when you report feeling well.
How long to wait before judging your biologic
Response timelines are built into the drugs themselves, so the honest answer depends on which biologic you are taking. Infliximab is given as an intravenous induction regimen at weeks 0, 2, and 6, followed by maintenance every 8 weeks. Ustekinumab starts with a single weight-based intravenous dose, with the first 90 mg subcutaneous maintenance dose 8 weeks later. Vedolizumab is dosed at weeks 0 and 2 with maintenance from week 6, and its label instructs prescribers to discontinue the drug in patients who show no evidence of therapeutic benefit by week 14. The 2025 American College of Gastroenterology ulcerative colitis guidelines advise reassessment at roughly 8 weeks after starting a new therapy. Judging a drug at week three usually judges it too early.
Signal one: symptoms, and what they cannot see
Symptoms are the signal you have every day, and they are the one most likely to mislead. In a study correlating patient-reported outcomes with the Simple Endoscopic Score for Crohn's Disease, stool frequency showed only a moderate correlation with endoscopic severity, and the relationship was far weaker in ileal disease than in colonic disease. Some people feel well while ulcers persist. Others have daily pain and diarrhea driven by bile acid malabsorption, strictures, or overlapping irritable bowel syndrome, none of which a biologic will fix. Symptom logs remain worth keeping because they capture trajectory, trigger the right tests, and give your gastroenterologist a record of stool frequency, urgency, rectal bleeding, pain, and fatigue that is more reliable than recall at a 20 minute appointment.
Signal two: calprotectin and CRP between visits
Fecal calprotectin measures inflammation in the gut directly, and it fills the gap between how you feel and what a scope would show. AGA guidelines for both diseases use a threshold of 150 micrograms per gram: in patients in symptomatic remission, a fecal calprotectin under 150 with a normal CRP argues against active inflammation and can spare a scope, while an elevated calprotectin or CRP in someone with moderate to severe symptoms supports acting on the result. For patients who recently adjusted therapy, the UC guideline notes a stricter cutoff near 50 may better predict endoscopic improvement. The AGA Crohn's guidance adds the key caveat: when symptoms and biomarkers disagree, that discordance is itself a reason for endoscopy.
Signal three: scope results as the reference standard
Endoscopy is the measurement everything else is validated against. In ulcerative colitis, the ACG target is endoscopic improvement defined as a Mayo endoscopic score of 0 or 1, and in Crohn's disease, biomarker performance is benchmarked against a Simple Endoscopic Score of 3 or higher. Evidence that biomarker-driven monitoring changes outcomes comes from the CALM trial, where patients whose treatment was escalated based on calprotectin of 250 or above, CRP of 5 or above, or clinical activity reached mucosal healing at week 48 in 46 percent of cases compared with 30 percent managed by symptoms alone. Scopes are infrequent, so the biomarkers and symptoms you log between them carry the monitoring load.
When the signals suggest the drug is failing
If inflammation persists after adequate induction, the next question is whether the drug is absent from your bloodstream or present and ineffective. AGA guidance suggests reactive therapeutic drug monitoring, meaning drug levels and antibody testing in patients with active disease on anti-TNF therapy, to guide whether to intensify or switch. Immunogenicity is a real mechanism of failure: the infliximab label reports antibodies to infliximab in roughly 10 percent of patients over one to two years, with higher rates after drug-free intervals longer than 16 weeks. Loss of response over time is common, and a meta-analysis of anti-TNF dose intensification found short-term response in 33 to 90 percent of patients, with further loss of response beyond six months in up to 64 percent.
A practical response record to bring to your appointment
The information your gastroenterologist needs to decide whether to continue, optimize, or switch is largely information only you can supply. A useful record covers three things.
- Symptom trend by week, including stool frequency, blood, urgency, pain, and fatigue
- Every calprotectin and CRP value with its date, so the direction of travel is visible
- Dosing history, including any late, missed, or self-delayed doses and any infusion reactions
That last item is easy to overlook and heavily influences interpretation. A calprotectin that climbed after a dose ran four weeks late points toward a different fix than one that climbed while you were perfectly adherent.
Judging a biologic is a matter of triangulation across time rather than a single verdict at a single visit. Symptoms tell you how you are living, calprotectin and CRP tell you whether inflammation is present between scopes, and endoscopy confirms what the other two only estimate. When all three agree, the decision is straightforward. When they disagree, that conflict is the finding, and it usually earns a closer look rather than a guess.
