IBD Disease Activity Scores: A Patient's Guide

IBD scores can look like a verdict: a number appears in a clinic note, colonoscopy report, or trial result, followed by words such as “mild” or “remission.” In reality, each score is a ruler designed for one part of inflammatory bowel disease. A symptom score records what you feel. An endoscopy score grades what a clinician sees. A biopsy score describes inflammation under a microscope. Imaging, laboratory, pediatric, postoperative, and quality-of-life tools answer still other questions. International treat-to-target guidance therefore uses symptoms, biomarkers, and endoscopic healing as complementary targets rather than asking one number to do every job (STRIDE-II consensus; CORE-IBD consensus).
The main families of IBD scores
Clinical activity indices combine symptoms and sometimes examination or laboratory findings. Crohn’s examples include the CDAI and Harvey-Bradshaw Index (HBI); UC examples include the Mayo score and SCCAI. Patient-reported outcomes are narrower: Crohn’s PRO-2 uses abdominal pain and stool frequency, while UC PRO-2 uses stool frequency and rectal bleeding. These tools are useful for tracking symptom burden, but symptoms can correlate imperfectly with inflammation seen at endoscopy (clinical scores and endoscopic remission study; PRO-2 and SES-CD study).
Endoscopic indices measure visible bowel injury. The SES-CD and CDEIS are used in Crohn’s disease; the Mayo endoscopic subscore and UCEIS are common in UC. Histology indices, including Nancy, Geboes, and Robarts, grade biopsy features that cannot be judged from symptoms alone. Capsule scores such as Lewis and CECDAI focus on the small bowel, while MaRIA and intestinal-ultrasound indices summarize imaging findings. These tools are not interchangeable because their inputs, scales, and intended uses differ (review of IBD activity indices; endoscopic activity review).
Other numbers describe context rather than current inflammation. The Montreal classification records Crohn’s age at diagnosis, location, and behavior. The Lémann Index estimates cumulative bowel damage. IBDQ and SIBDQ measure health-related quality of life. A person can have a stable Montreal classification, established structural damage, improved inflammation, and continuing fatigue at the same time. Those results are not contradictions. They describe different layers of the disease.
Which scores use lab results?
Most familiar IBD scores do not calculate CRP or fecal calprotectin into their totals. HBI, SCCAI, PRO-2, SES-CD, UCEIS, PUCAI, and the Mayo score keep laboratory results separate. CDAI is an important exception because hematocrit is one of its eight weighted inputs. The original PCDAI for pediatric Crohn’s includes hematocrit, ESR, and albumin, alongside symptoms, examination, and growth information (original CDAI development; PCDAI laboratory-marker study). Some hospital prediction tools also use labs, but they answer a more specific question, such as the chance that acute severe UC will not respond to intravenous steroids.
CRP and calprotectin are better understood as separate biomarkers. CRP reflects a systemic inflammatory response, while fecal calprotectin reflects intestinal neutrophilic inflammation. Neither identifies the cause by itself, and results must be interpreted with symptoms and clinical context. The American Gastroenterological Association recommends combining symptoms with biomarkers in Crohn’s monitoring rather than relying on symptoms alone (AGA Crohn’s biomarker guidance). A lab can therefore influence a treatment decision without being a mathematical part of the score written beside it.
How to interpret a score in your record
Start by capturing the exact name, version, date, and inputs. “Mayo 3” could mean an endoscopic subscore of 3 or a total on a different Mayo version. A partial Mayo score cannot be compared directly with a full Mayo total. Likewise, HBI and CDAI may move together but are not the same scale. Ask what the score was intended to measure and whether the change is large enough to matter for that tool.
Then place it on a timeline with stool frequency, bleeding, pain, weight, medications, CRP, calprotectin, imaging, and scopes. A falling symptom score with persistently high calprotectin deserves a different conversation from improving symptoms and improving biomarkers. New severe pain, heavy bleeding, fainting, dehydration, fever, or inability to keep fluids down should prompt urgent clinical advice rather than home score calculation. The most useful IBD “dashboard” is not one perfect number. It is a dated set of measures that lets you and your care team see where the layers agree, where they do not, and what needs checking next. Keep the original report and individual components whenever possible, since a total alone can conceal which symptom, lesion, lab, or segment changed.
Record the score date separately from the visit date when they differ, and keep the original report for later comparison.
