Waking up to cramping, urgency, and four trips to the bathroom before breakfast raises the same question for almost everyone with inflammatory bowel disease: is this a flare, a stomach bug, or something I ate? The honest answer is that symptoms alone cannot settle it. Diarrhea, abdominal pain, nausea, and fatigue appear in all three situations, and the overlap is the reason gastroenterologists order stool tests rather than guessing. What symptom patterns and timing can do is narrow the possibilities, tell you which questions to ask, and make clear when the answer needs to come from a clinician rather than from waiting it out.

Why the Three Possibilities Overlap So Much

The core problem in an IBD flare versus infection comparison is that inflamed bowel and infected bowel produce nearly identical complaints. The most common symptoms of Crohn's disease are diarrhea, abdominal cramping, and weight loss, with fever, nausea, and fatigue also common. Viral gastroenteritis produces watery diarrhea, abdominal cramping, nausea or vomiting, and sometimes fever. Food poisoning produces diarrhea or bloody diarrhea, vomiting, abdominal pain, fever, and headache.

There is a fourth possibility worth naming, because it is common and frequently misread as a flare. Roughly a third of people with IBD in remission report symptoms meeting criteria for irritable bowel syndrome, and about a quarter still do when remission is confirmed endoscopically. Those symptoms are real, but the mechanism is different from active inflammation and so is the treatment.

A Practical Comparison of the Patterns

Timing is the most useful single clue. Norovirus symptoms typically begin 12 to 48 hours after exposure, and lactose-related symptoms appear within a few hours of consuming milk products. Inflammation rarely switches on that fast.

What Points Toward an Infection

Infection deserves serious consideration whenever flare symptoms arrive suddenly, especially after antibiotics, hospitalization, travel, or contact with someone who was recently ill. This is not a minor academic distinction. The American Gastroenterological Association advises that clinicians should test patients presenting with a flare of underlying IBD for Clostridioides difficile infection, and its 2026 practice update recommends a multistep toxin-based assay rather than PCR alone, because colonization is common in IBD and a positive PCR by itself does not prove active infection. That update also advises continuing biologics and immunomodulators alongside antibiotic treatment.

The stakes justify the testing. In a matched cohort study, enteric infection identified at the time of an IBD flare was independently associated with worse two-year outcomes, including steroid dependence, colectomy, or treatment escalation, with C. difficile carrying the strongest association. Treating an infection as a flare and escalating immunosuppression without testing can make the situation worse.

What Points Toward a Food Reaction

Food reactions tend to be reproducible, time-locked, and self-limiting. Symptoms start within hours of a particular meal, ease once the food has passed through, and return when that food comes back. Bloating, gas, cramping, and loose stools dominate, while blood, fever, and progressive weight loss point elsewhere. Lactose is a common culprit, producing bloating, gas, diarrhea, nausea, and abdominal pain in proportion to the amount consumed.

Fermentable carbohydrates are another. In a randomized trial in quiescent IBD, 52% of patients on a low FODMAP diet reported adequate relief of persistent gut symptoms compared with 16% on a control diet, while markers of inflammation did not differ between groups. That result captures the distinction precisely: food can drive real symptoms without driving inflammation, which is why dietary change should not replace an assessment of disease activity.

Why Objective Testing Settles the Question

Because the three explanations look alike from the outside, guidelines lean on stool and blood markers rather than symptom scores. AGA guidance for ulcerative colitis suggests using fecal calprotectin above 150 micrograms per gram, elevated fecal lactoferrin, or elevated C-reactive protein to inform treatment decisions in patients with moderate to severe symptoms, and values below that threshold to help rule out active inflammation in those who feel well. Parallel AGA guidance for Crohn's disease reaches similar conclusions and notes that disagreement between symptoms and biomarkers is a reason to look directly with endoscopy. A stool pathogen panel and C. difficile testing answer the infection question at the same time, which is why many gastroenterology teams order both together at the start of an unexplained symptom change.

Symptoms That Need Urgent Assessment

Some presentations should bypass the sorting exercise entirely. In ulcerative colitis, acute severe disease is defined as more than six bloody bowel movements a day plus at least one systemic sign such as fever above 37.8 degrees Celsius, heart rate above 90 beats per minute, hemoglobin below 10.5 grams per deciliter, or markedly raised inflammatory markers. That combination is a reason for same-day contact and often hospital admission under the 2025 American College of Gastroenterology ulcerative colitis guideline. Anti-motility drugs such as loperamide should be avoided in severe colitis because they may precipitate toxic megacolon.

Obstruction is the other emergency pattern, particularly in Crohn's disease with narrowed segments. Abdominal pain with vomiting and no passage of stool, fluid, or gas warrants immediate medical attention. Separately, food poisoning guidance advises urgent care for six or more loose stools in a day, diarrhea beyond three days, severe abdominal pain, bloody or tarry stools, or signs of dehydration.

Sorting a flare from an infection from a food reaction is a process of narrowing, followed by testing. Onset speed, blood, vomiting, recent exposures, and whether symptoms track a specific food all shift the odds, and none of them close the case. The practical move during an episode is to record what is happening while it is happening, including stool frequency, blood, temperature, what was eaten, and any recent antibiotics or sick contacts, then bring that record to a clinician who can pair it with stool and blood testing.
