Normal Calprotectin but Ongoing IBD Symptoms: Inflammation, IBS, Infection, and Other Possibilities

A stool test came back in the normal range, and the report reads like good news. Meanwhile the urgency, the cramping, and the trips to the bathroom have not changed at all. If you have a normal calprotectin but still have symptoms, you are in one of the most common and most frustrating positions in inflammatory bowel disease care. Fecal calprotectin is a protein released by neutrophils, the white blood cells that flood the gut lining during active inflammation, and it is a good test for what it measures. It is a single number from a single sample, and there are several well-documented reasons it can look reassuring while you feel unwell.
What a normal calprotectin result does and does not rule out
Fecal calprotectin performs well at separating inflammatory bowel disease from irritable bowel syndrome, with a pooled sensitivity of 85.8% and specificity of 91.7% in a 2023 meta-analysis. That leaves a meaningful minority of people with genuine inflammation whose test does not flag it. Guideline bodies treat the number as one input rather than a verdict. The American Gastroenterological Association guideline on biomarkers in Crohn's disease uses a threshold of 150 micrograms per gram, and it is explicit that when someone has ongoing symptoms and normal biomarkers, endoscopic assessment is preferred over changing treatment empirically. The companion guideline for ulcerative colitis makes the same point, suggesting endoscopic assessment for patients with moderate to severe symptoms and normal biomarkers. A normal result is most trustworthy in a person who feels well, where it can reasonably rule out active inflammation and spare a colonoscopy. In a person who feels unwell, it carries much less weight.
Calprotectin can miss inflammation higher in the small bowel
Where the inflammation sits matters. Calprotectin has to survive transit through the entire gut to reach the stool, and disease confined to the upper small intestine produces less of it. A capsule endoscopy study measuring calprotectin against inflammation at each level of the small bowel found that levels rose steadily from the proximal small bowel toward the colon, and that the cutoff needed to detect mild proximal inflammation was 77 micrograms per gram, far below the standard 150 threshold. In practical terms, a result of 90 or 120 could represent real jejunal or upper ileal Crohn's disease while being reported as normal. This is why gastroenterologists evaluating persistent small bowel symptoms often move to magnetic resonance enterography, computed tomography enterography, intestinal ultrasound, or capsule endoscopy rather than repeating the stool test.
How the sample was collected can move the number
Calprotectin is not evenly distributed through a bowel movement, and it fluctuates through the day. A prospective study of people with inflammatory bowel disease found clinically significant variability of 8% to 23% within a single stool sample, depending on where in the specimen the material was taken and the cutoff applied, and 13% to 26% between different bowel movements in the same 24 hours. The authors concluded that the first sample of the day gave the most reliable reading. Watery stool from a flare can also dilute the measurement. If your result sits close to the threshold, the timing and handling of that particular sample are worth mentioning to your gastroenterologist, and a repeat test collected in the morning may give a clearer answer than the original one did.
Symptoms that behave like irritable bowel syndrome
Inflammation and symptoms come apart often enough that gastroenterology has a name for the pattern. In a meta-analysis of 3,169 people with inflammatory bowel disease in remission, 32.5% reported symptoms meeting diagnostic criteria for irritable bowel syndrome, and 23.5% still did when remission was confirmed endoscopically rather than by symptom scores alone. Prevalence was higher in Crohn's disease at 36.6% than in ulcerative colitis at 28.7%. The mechanisms are thought to involve visceral hypersensitivity and altered gut motility left behind after inflammation resolves, sometimes compounded by scarring in previously inflamed segments. This does not mean the symptoms are imagined or minor. It means the treatment that helps is likely to be different from escalating your biologic, drawing instead on dietary approaches, gut-directed neuromodulators, and psychological therapies for the gut-brain axis.
Strictures, surgery, and changed anatomy
Structural problems can produce severe symptoms with little active inflammation to detect. Narrowing of the bowel develops in roughly 35% of people with Crohn's disease within ten years of diagnosis, and once a stricture is predominantly fibrotic, the cramping, bloating, and post-meal pain it causes reflect obstruction rather than active disease. Prior surgery changes things in a different way. Removing the terminal ileum impairs bile acid reabsorption, and bile acids reaching the colon drive secretory diarrhea. One review reports postoperative diarrhea in 79% of patients after ileal resection of more than 10 centimeters, a picture that responds to bile acid sequestrants rather than to immunosuppression. Neither scenario reliably raises calprotectin.
Infection and bacterial overgrowth
Infection deserves early consideration whenever diarrhea persists. The AGA advises testing for Clostridioides difficile in anyone presenting with a flare of underlying inflammatory bowel disease, and re-testing if symptoms return after treatment. Because carriage without active infection is common in this population, the 2021 American College of Gastroenterology guideline supports testing algorithms that combine a highly sensitive and a highly specific method rather than relying on a single assay. Small intestinal bacterial overgrowth is another candidate, found in 22.3% of people with inflammatory bowel disease across eleven studies, with fibrostenosing disease and prior ileocecal valve resection standing out as risk factors. Both conditions produce bloating, urgency, and loose stool that closely mimic a flare.
Questions worth bringing to your appointment
Coming in with specifics changes the conversation. Bring the numeric calprotectin value and the date, the lab's stated reference range, whether the sample was your first of the day, and a record of how your symptoms have tracked over recent weeks.
- Does the number fit the disease location I have, particularly if my Crohn's involves the upper small bowel?
- Should we look directly with endoscopy or cross-sectional imaging rather than repeat the stool test?
- Have we ruled out infection, bacterial overgrowth, bile acid diarrhea, and stricture as explanations?
A normal calprotectin narrows the list of explanations without emptying it. The result tells your care team that colonic neutrophil activity was low when your sample was collected, which is useful information and a reason to examine the alternatives rather than assume nothing is wrong. Persistent symptoms with normal biomarkers is a recognized scenario with an established workup, and the guidelines point toward direct visualization and a systematic search for non-inflammatory causes.
