UCDAI Score: How Ulcerative Colitis Activity Is Measured

The Ulcerative Colitis Disease Activity Index, or UCDAI, is also called the Sutherland Index and sometimes simply the Disease Activity Index in older treatment studies. It scores stool frequency, rectal bleeding, mucosal appearance, and a physician’s assessment, each from 0 to 3. The total resembles the full Mayo score, but the instruments and endpoint definitions should not be assumed identical. UCDAI contains no CRP, ESR, hemoglobin, albumin, or fecal calprotectin (IBD activity-index review).

The four UCDAI components

Stool frequency is judged relative to the person’s normal number. Rectal bleeding ranges from none to passing blood without stool. Mucosal appearance is graded during endoscopy, with findings such as friability, exudate, spontaneous bleeding, and ulceration influencing the category. The physician rating summarizes overall disease activity based on clinical judgment.

Because the total can range from 0 to 12, it is tempting to apply Mayo interpretation tables. That shortcut can fail. The original wording of endoscopic and global-assessment categories, trial adaptations, and definitions of response or remission may differ. When UCDAI appears in a paper, record the stated formula and endpoint. “DAI” can also be a generic phrase, so confirm that it means the Sutherland instrument.

The endoscopic component means a complete UCDAI cannot be collected at every routine visit. A symptom-only diary may resemble part of the score but is not the total. If a report provides only a global value, ask for the four components because a change driven by bleeding has a different meaning from a change driven by endoscopic appearance.

UCDAI versus the Mayo family

Both UCDAI and full Mayo combine stool frequency, bleeding, endoscopy, and clinician assessment. Partial Mayo removes endoscopy, while modified Mayo removes the physician global assessment. These similar structures help explain why the names are sometimes blurred in summaries, but the safest approach is to preserve the instrument used (Mayo-score comparison).

Newer trials may use modified Mayo or a two-item patient-reported endpoint. UC PRO-2 uses stool frequency and rectal bleeding only. It can document symptomatic remission without a scope, but it cannot replace endoscopic assessment (UC PRO-2 study). UCEIS is another separate endoscopic score based on vascular pattern, bleeding, and erosions or ulcers (endoscopic activity review).

Where lab values belong

Routine labs do not enter UCDAI, but they may change how the result is interpreted. CRP can reflect systemic inflammation, fecal calprotectin reflects intestinal neutrophilic inflammation, hemoglobin can reveal anemia, and albumin can reflect inflammation or nutritional and protein-loss effects. AGA guidance recommends using biomarkers with symptoms during UC monitoring rather than treating either layer as sufficient in every situation (AGA UC biomarker guidance).

Keep the exact score name, component values, diary dates, scope report, and trial definition. Add medications, CRP, calprotectin, CBC, and albumin from the same period. Do not use an online UCDAI result to start or stop therapy. Seek prompt clinical help for heavy bleeding, fainting, fever, severe abdominal pain or swelling, dehydration, or rapidly worsening stool frequency. UCDAI remains useful for understanding older evidence, provided its name, components, and laboratory context are not lost in translation.

Questions that make the number more useful

Before comparing this result with an older one, confirm that both records used the same named instrument, version, scoring range, and assessment window. Ask whether the total represents the worst segment, a sum across segments, an average, or a single day. If a clinician or study used a modified version, record exactly what was removed or added. Also check whether treatment, bowel preparation, infection, missed diary entries, or incomplete examination affected the inputs. A small apparent change may reflect measurement conditions rather than a meaningful change in disease.

At a visit, useful questions include: What domain does this score measure? Which component drove the result? What would count as response or remission in this setting? Does the number need confirmation with blood work, stool testing, endoscopy, biopsy, or imaging? Is the goal to follow symptoms, current inflammation, structural damage, treatment response, or quality of life? Asking these questions turns an unfamiliar total into a specific clinical conversation.

For personal tracking, avoid combining unlike scores on one graph. Keep each instrument in its own series and place objective tests on parallel timelines. Save the source report because the component details may matter later. Note both the sample or procedure date and the date you received the result. Trends are easiest to interpret when the method and timing stay consistent. If the number conflicts with how you feel or with another test, bring the disagreement to your care team instead of choosing the result that seems most reassuring. Discordance is often the reason to look more closely.
