Ulcerative Colitis PRO-2 Score: Blood and Stool Frequency

Ulcerative colitis PRO-2 is a two-item patient-reported outcome based on stool frequency and rectal bleeding. The items come from the Mayo framework but leave out endoscopy and the physician global assessment. PRO-2 is used frequently in medication trials to describe symptomatic response or remission. It contains no urgency item, nighttime-stool item, CRP, fecal calprotectin, or biopsy result. A low score can show meaningful symptom control, but it cannot prove mucosal healing (UC PRO-2 study; CORE-IBD).

How stool frequency and bleeding are scored

Stool frequency is categorized relative to the person’s normal number of stools, usually on the Mayo 0-to-3 scale. Rectal bleeding is likewise graded from none to blood passed without stool. Trials collect these items in daily electronic diaries and summarize a prespecified window. A result may be reported as a sum, as separate subscores, or as meeting a categorical endpoint.

“PRO-2 remission” is not one universal number. Protocols may require a stool-frequency score of 0 or 1 and a rectal-bleeding score of 0, sometimes with additional rules about change from baseline. Missing diary days and averaging methods matter. When comparing medications, read each study’s exact definition and assessment time point.

Crohn’s disease also has a PRO-2, but it combines abdominal pain and very soft or liquid stool frequency. The shared name does not mean the inputs or remission thresholds are interchangeable.

PRO-2 versus partial Mayo and SCCAI

Partial Mayo adds a physician global assessment to stool frequency and bleeding. SCCAI asks about daytime and nighttime stools, urgency, blood, well-being, and extraintestinal symptoms. UC PRO-2 is narrower and fully patient-reported. It is efficient and focused on two core symptoms, but it can miss burdens such as urgency, incontinence, nocturnal waking, fatigue, or pain (Mayo-score comparison; IBD activity-index review).

Patient-reported remission is not the same as endoscopic improvement. Research has tested the relationship between PRO-2 and mucosal healing, but the association does not make one a substitute for the other (UC PRO-2 study). Trials may therefore require both a symptom endpoint and an endoscopic endpoint.

Labs are co-evidence, not inputs

CRP and fecal calprotectin are separate biomarkers. AGA guidance recommends combining symptoms and biomarkers in UC monitoring and using endoscopic assessment when results leave clinically important uncertainty (AGA UC biomarker guidance). STRIDE-II similarly separates symptom response, biomarker normalization, and endoscopic healing (STRIDE-II).

Track stool frequency against a stable personal baseline and record bleeding daily using consistent definitions. Keep urgency and nighttime stools too, even though they do not enter PRO-2, because they matter clinically. Add medication dates, CRP, calprotectin, CBC, albumin, endoscopy, and biopsy results. Do not change treatment from a self-calculated endpoint. Seek prompt advice for rapidly worsening frequency, heavy blood loss, fever, fainting, severe pain or swelling, or dehydration. PRO-2 gives two patient-reported symptoms formal weight; the surrounding evidence shows whether symptom control and inflammatory control are arriving together.

Questions that make the number more useful

Before comparing this result with an older one, confirm that both records used the same named instrument, version, scoring range, and assessment window. Ask whether the total represents the worst segment, a sum across segments, an average, or a single day. If a clinician or study used a modified version, record exactly what was removed or added. Also check whether treatment, bowel preparation, infection, missed diary entries, or incomplete examination affected the inputs. A small apparent change may reflect measurement conditions rather than a meaningful change in disease.

At a visit, useful questions include: What domain does this score measure? Which component drove the result? What would count as response or remission in this setting? Does the number need confirmation with blood work, stool testing, endoscopy, biopsy, or imaging? Is the goal to follow symptoms, current inflammation, structural damage, treatment response, or quality of life? Asking these questions turns an unfamiliar total into a specific clinical conversation.

For personal tracking, avoid combining unlike scores on one graph. Keep each instrument in its own series and place objective tests on parallel timelines. Save the source report because the component details may matter later. Note both the sample or procedure date and the date you received the result. Trends are easiest to interpret when the method and timing stay consistent. If the number conflicts with how you feel or with another test, bring the disagreement to your care team instead of choosing the result that seems most reassuring. Discordance is often the reason to look more closely.
