Celltrion CONNECT reviews benefit information, supports administrative access work, and coordinates with the clinic and specialty pharmacy. Clinical documentation and treatment decisions remain with the prescriber.

Provide dose, diagnosis, prior infliximab treatment, chart notes, and plan-requested records.
Clinics can enroll through the Celltrion CONNECT HCP portal, fax the completed form, or e-prescribe to Phyz using NCPDP 5928809. Include insurance cards, demographics, chart notes, clinical records, patient authorization, and prescriber signature.
