What it does
The hub uses the signed enrollment form and insurance information to research benefits and provide administrative support to the patient and clinic. Clinical submissions remain the prescriber's responsibility.

Attach current medical and pharmacy cards.
Complete the product, dose, diagnosis, and prescriber sections and submit plan-requested records.
The patient can create an account, but the clinic should submit the official enrollment and prescription form with authorization, insurance cards, prescription, and requested clinical details.
