Biologic Trough Levels and Antibodies: A Patient Guide to Therapeutic Drug Monitoring

When symptoms return on a biologic that used to work, gastroenterologists often order a blood test before changing anything. That test measures biologic trough levels and antibodies, and the results arrive as two numbers with a reference range that means little without context. Understanding what therapeutic drug monitoring in IBD measures, why the timing of the blood draw relative to your last dose determines whether the number can be interpreted, and what each combination of results implies will help you take part in the decision that follows.
What a Trough Level and Antibody Test Measure
Therapeutic drug monitoring uses a single blood sample to answer two questions. The first is how much active biologic is circulating in your blood, reported in micrograms per milliliter. The second is whether your immune system has produced antidrug antibodies, proteins that bind the biologic and clear it from circulation before it reaches inflamed tissue. Commercial assays for infliximab report both the drug concentration and the anti-infliximab antibody level from the same draw, and the two results are meant to be interpreted together.
Antibody formation, known as immunogenicity, is common with anti-tumor necrosis factor drugs. In the PANTS study, a prospective cohort of anti-TNF-naive patients with Crohn's disease, antidrug antibodies developed in 62.8% of patients on infliximab and 28.5% of patients on adalimumab by week 54. Low drug concentration, largely driven by immunogenicity, was the only factor independently associated with primary non-response in that cohort.
Why the Timing of the Blood Draw Decides the Answer
A trough level is the concentration at the lowest point of the dosing cycle, measured immediately before the next scheduled dose. For infliximab, the sample should be collected just prior to the next infusion. For subcutaneous drugs such as adalimumab, the same principle applies to the day the next injection is due. Published target ranges were derived from samples drawn at that specific point, so a level measured at any other time in the cycle cannot be compared to them.
This makes your dose history part of the test result. A blood draw scheduled for a trough is only a trough if the previous dose was given on the expected date. A delayed infusion, a missed injection, or a dose given early all shift the concentration curve, and a level drawn without that information can look falsely low or falsely reassuring. Recording the exact date of each dose, along with any interval changes, gives the lab number the anchor it needs.
What the Numbers Generally Mean
Target concentrations differ by drug, by dosing phase, and by the outcome being aimed for. For infliximab in maintenance, commonly cited targets fall in the range of 5 to 10 micrograms per milliliter, with adalimumab maintenance targets of 8 to 12 micrograms per milliliter. The American Gastroenterological Association applies a simpler threshold for reactive testing during active disease, suggesting infliximab above 5 micrograms per milliliter. Post-induction targets run higher, and in PANTS the week 14 concentrations best associated with remission at both week 14 and week 54 were 7 mg/L for infliximab and 12 mg/L for adalimumab.
Higher adalimumab concentrations have similarly been associated with sustained remission and normal C-reactive protein. These are population averages rather than personal cutoffs, which is why a result slightly below a published number is a prompt for discussion rather than an automatic instruction to switch drugs.
Reading Drug Level and Antibody Results Together
Four patterns emerge, and each points toward a different action. An adequate drug level with no meaningful antibodies suggests the medication is reaching its target and that ongoing symptoms may be driven by something outside that pathway, so escalating the dose is unlikely to help. A low drug level without antibodies usually reflects rapid clearance, high inflammatory burden, or missed doses, and typically supports dose escalation or interval shortening rather than a change of drug.
A low drug level with high antibodies points toward neutralizing immunogenicity, where adding an immunomodulator or switching therapy is the usual response. Low-titer antibodies detected alongside a preserved drug level are often transient and may resolve without any change, which is one reason repeat testing is sometimes preferred to an immediate switch.
When Guidelines Recommend Testing
The AGA suggests reactive therapeutic drug monitoring to guide treatment changes in adults with active IBD on anti-TNF agents, and makes no recommendation for or against routine proactive monitoring in patients whose disease is quiet. The updated 2025 ACG ulcerative colitis guideline takes a similar position, suggesting measurement of serum drug levels and antidrug antibodies in patients losing response to anti-TNF therapy while noting insufficient evidence for routine proactive monitoring in unselected patients in remission, and insufficient evidence to support measuring concentrations of vedolizumab, ustekinumab, guselkumab, mirikizumab, or risankizumab.
Evidence for proactive monitoring continues to accumulate. Exploratory analyses of the NOR-DRUM trials found antidrug antibodies in 24% of patients and linked them to lower remission rates, more disease worsening, sharply increased infusion reactions, and more discontinuation, with proactive monitoring reducing those consequences.
Lowering the Chance of Antibodies
Combining an anti-TNF drug with an immunomodulator reduces antibody formation. In the SONIC trial, patients with Crohn's disease treated with infliximab plus azathioprine reached corticosteroid-free clinical remission at week 26 more often than those on infliximab alone, 56.8% versus 44.4%, and antibodies to infliximab were far less frequent with combination therapy. PANTS found that concomitant thiopurine or methotrexate reduced the hazard of developing antidrug antibodies by roughly 60% for infliximab and 56% for adalimumab. Consistent dosing matters as well, since gaps in exposure create the low-drug conditions in which antibodies form most readily. That relationship runs in both directions in the PANTS cohort, where suboptimal week 14 concentrations predicted later antibody formation and antibodies in turn predicted lower concentrations afterward.
What Belongs Alongside the Result
A drug level answers a pharmacology question, and it does not tell your team whether the bowel is inflamed. The AGA recommends combining symptoms with objective biomarkers, using thresholds of fecal calprotectin around 150 micrograms per gram and CRP within the normal range in Crohn's disease, with comparable guidance in ulcerative colitis. Symptoms with normal biomarkers and an adequate drug level point to a cause other than active inflammation. Elevated biomarkers with a low drug level and no antibodies make a dosing problem the leading explanation.
Before agreeing to a treatment change based on a single result, it is reasonable to ask when the sample was drawn relative to your last dose, whether the antibody result was low titer or high, and whether repeating the test after a dose adjustment would clarify the picture. A trough level is a snapshot of one moment in one dosing cycle, and its usefulness depends on how accurately that moment can be placed in the history of your treatment.
