High Calprotectin but You Feel Fine: What the Mismatch Can Mean

You feel good. Your bowel movements are predictable, your energy is back, and you have not thought about a bathroom map in months. Then a stool test result lands in your patient portal with a number several times higher than the reference range, flagged in red. High calprotectin with no symptoms is one of the more disorienting experiences in inflammatory bowel disease monitoring, because it asks you to take seriously a problem your body is not reporting. The mismatch is common, it is well described in the research literature, and it has a defined set of possible explanations. Understanding which explanation applies to you is the work you and your gastroenterologist do next.
What the calprotectin test actually measures
Calprotectin is a protein released by neutrophils, a type of white blood cell that migrates into the intestinal wall when there is inflammation and then passes into stool. The calprotectin stool test measures how much of that protein is present, which makes it a proxy for how many inflammatory cells are currently trafficking through your gut lining. It measures cellular activity in the bowel wall, and cellular activity is not the same thing as the sensation of being unwell.
That distinction is the whole reason the test exists. Symptoms are filtered through pain perception, gut motility, diet, stress, and the anatomy of whichever segment is affected. Calprotectin skips all of that and reports on the tissue directly. The American College of Gastroenterology's 2025 ulcerative colitis guideline gives a strong recommendation for using fecal calprotectin to assess response to therapy, evaluate suspected relapse, and monitor during maintenance, precisely because it sees things symptoms miss.
Why symptoms and inflammation disagree so often
Feeling well while inflammation persists is the norm rather than the exception. A prospective study of 152 people with IBD who underwent surveillance colonoscopy while in clinical remission found that 68 percent had some form of mucosal inflammation despite reporting no active disease. Roughly half of those had visible endoscopic lesions and the other half had inflammation detectable only under a microscope.
This gap is why treatment targets in IBD shifted away from symptoms alone. The STRIDE-II consensus from the International Organization for the Study of IBD established normalization of calprotectin and serum inflammatory markers as a short-term treatment target, with endoscopic healing as the longer-term goal. The CALM trial tested whether acting on that idea changes outcomes. Patients whose treatment was escalated based on calprotectin at or above 250 micrograms per gram alongside symptoms reached mucosal healing at week 48 at a rate of 46 percent, compared with 30 percent for those managed on symptoms and steroid use alone. Acting on a number the patient cannot feel produced better tissue outcomes.
What one elevated result predicts, and what it does not
An elevated result raises your statistical risk of relapse without guaranteeing one. A meta-analysis of 14 studies covering 1,110 adults with ulcerative colitis in remission found calprotectin predicted relapse with pooled sensitivity of 0.75 and specificity of 0.77. Useful, and far from perfect.
Repetition sharpens the signal considerably. A systematic review of six prospective studies of calprotectin monitoring in asymptomatic IBD patients found that people with repeated elevated values had a 53 to 83 percent probability of relapsing within the following two to three months, while those with repeated normal values had a 67 to 94 percent probability of staying in remission. One high result is a question. Two consecutive high results is closer to an answer.
Things besides IBD that raise calprotectin
Several ordinary exposures push the number up in people whose disease is genuinely quiet. Common medications are among the most underappreciated. An observational study of patients with functional bowel disorders found that those prescribed proton pump inhibitors had median calprotectin of 66 versus 23 micrograms per gram, and those prescribed nonsteroidal anti-inflammatory drugs had 57 versus 25. Ibuprofen for a headache or omeprazole for reflux can move a result across a threshold on its own.
Beyond medications, bacterial gastroenteritis, celiac disease, and colorectal neoplasia can all elevate calprotectin, which is why additional testing is often needed before an elevated result is attributed to IBD. A recent stomach bug or a course of naproxen belongs in the conversation with your care team.
The number itself is noisier than it looks
Two results from the same person in the same week can differ substantially without any change in disease. A prospective study of 50 adults with IBD who sampled multiple portions of stool across a 24 hour period found variability of 8 to 23 percent within a single bowel movement and 13 to 26 percent between different bowel movements on the same day. The authors concluded the first sample of the day gave the most reliable reading.
Laboratory platform matters too. A proficiency testing analysis found calprotectin assays from different manufacturers produced Deming regression slopes spanning 0.37 to 1.91, meaning the same specimen can return values that differ by several fold depending on which assay ran it. Comparing a result from one lab against a result from another can manufacture a trend that does not exist. Ask whether your samples have been run on the same platform before you interpret a rise.
What guidelines say should happen next
Society guidance is deliberately measured about acting on an isolated elevation in someone who feels well. The AGA guideline on biomarkers in ulcerative colitis uses 150 micrograms per gram as the threshold below which active inflammation can be ruled out in symptomatic remission, and recommends endoscopic assessment rather than empiric treatment changes when markers exceed it. The companion Crohn's disease guideline applies the same 150 threshold, with a stricter cutoff of 50 within twelve months of surgery. Repeat testing in three to six months is a reasonable middle path when the elevation is modest, and it is well supported by the two-consecutive-values evidence.
Useful questions for the appointment include which assay and threshold your lab uses, whether any medication or recent infection could explain the result, and whether the plan is to repeat, image, scope, or watch. Bringing a dated list of your results alongside your own record of symptoms, medications, and stool changes turns an isolated red flag into a trend your clinician can read. The mismatch between how you feel and what your stool test says is information, and information you have tracked over time is worth considerably more than a single number in a portal.
