Pouchitis PDAI Score After UC Surgery: What It Measures

The Pouchitis Disease Activity Index, or PDAI, is a composite score used after ileal pouch-anal anastomosis, often called J-pouch surgery. It combines symptoms, pouchoscopy findings, and biopsy findings, with up to 6 points in each domain for an 18-point total. A modified PDAI, or mPDAI, omits histology and ranges to 12. Neither version calculates CRP, fecal calprotectin, or fecal lactoferrin into the total. A commonly used PDAI threshold is 7 or more, but pouchitis should not be diagnosed from that cutoff alone (PDAI systematic review; pouchitis clinical review).
The three PDAI domains
The clinical domain considers stool frequency compared with the person’s usual postoperative pattern, rectal bleeding, urgency or abdominal cramping, and fever. That baseline is crucial: a healthy pouch normally produces more frequent stools than an intact colon, so a universal bowel-movement number would mislead.
The endoscopic domain grades edema, granularity, friability, loss of vascular pattern, mucous exudate, and ulceration during pouchoscopy. The histologic domain grades neutrophilic infiltration and ulceration in biopsy samples. Symptoms can therefore be substantial without matching the endoscopic or microscopic domains, and visible inflammation can occur with limited symptoms (pouchitis clinical review).
The mPDAI adds clinical and endoscopic domains but removes histology. A value from the 12-point modified scale cannot be interpreted as though it came from the 18-point full scale. Save the instrument name and each domain score.
Why a cutoff is not a complete diagnosis
Studies often use PDAI 7 or greater to define pouchitis, yet a systematic review found wide variation in diagnostic criteria, score use, and outcome definitions across the literature (PDAI systematic review). A total can also hide discordance: six symptom points plus limited scope activity is different from mild symptoms plus marked endoscopic and histologic activity.
Other conditions can cause increased frequency, urgency, bleeding, or pain after pouch surgery. Possibilities include infection, cuffitis, Crohn’s-like disease of the pouch, anastomotic or mechanical problems, pelvic-floor dysfunction, and irritable pouch syndrome. Evaluation may require stool testing, examination, pouchoscopy with biopsies, imaging, or functional testing depending on the pattern (pouchitis clinical review).
Where biomarkers fit
CRP is a systemic marker and may be insensitive to limited pouch inflammation. Fecal calprotectin and lactoferrin have been studied as noninvasive pouch biomarkers, but neither is part of PDAI arithmetic. Results can support a decision to investigate, not replace the clinical, endoscopic, and histologic assessment.
Track your usual pouch frequency before symptoms begin, then record frequency, nighttime stools, urgency, leakage, blood, cramping, fever, antibiotics, and hydration. Keep the full or modified PDAI version, pouchoscopy and biopsy reports, stool-test results, CRP, calprotectin, and imaging together. Contact the pouch team promptly for severe pain, fever, heavy bleeding, repeated vomiting, inability to maintain hydration, or difficulty passing pouch output. PDAI is a structured framework for combining three domains. Its cutoff is evidence, not a stand-alone explanation for every pouch symptom.
Questions that make the number more useful
Before comparing this result with an older one, confirm that both records used the same named instrument, version, scoring range, and assessment window. Ask whether the total represents the worst segment, a sum across segments, an average, or a single day. If a clinician or study used a modified version, record exactly what was removed or added. Also check whether treatment, bowel preparation, infection, missed diary entries, or incomplete examination affected the inputs. A small apparent change may reflect measurement conditions rather than a meaningful change in disease.
At a visit, useful questions include: What domain does this score measure? Which component drove the result? What would count as response or remission in this setting? Does the number need confirmation with blood work, stool testing, endoscopy, biopsy, or imaging? Is the goal to follow symptoms, current inflammation, structural damage, treatment response, or quality of life? Asking these questions turns an unfamiliar total into a specific clinical conversation.
For personal tracking, avoid combining unlike scores on one graph. Keep each instrument in its own series and place objective tests on parallel timelines. Save the source report because the component details may matter later. Note both the sample or procedure date and the date you received the result. Trends are easiest to interpret when the method and timing stay consistent. If the number conflicts with how you feel or with another test, bring the disagreement to your care team instead of choosing the result that seems most reassuring. Discordance is often the reason to look more closely.
