Robarts Histopathology Index for UC Biopsies

The Robarts Histopathology Index, or RHI, is a validated ulcerative colitis biopsy score used mainly in trials and specialist reporting. It combines four microscopic features: chronic inflammatory infiltrate, neutrophils in the lamina propria, neutrophils in the epithelium, and erosion or ulceration. The weighted result ranges from 0 to 33, with higher scores indicating more histological activity. Symptoms, endoscopic appearance, CRP, and fecal calprotectin are not formula inputs (RHI development study; ECCO histology overview).

The four features behind the RHI

Chronic inflammatory infiltrate describes background immune-cell density in the lamina propria. Lamina propria neutrophils record acute inflammatory cells in the tissue surrounding crypts. Epithelial neutrophils capture cryptitis or crypt abscess activity. Erosion or ulceration records loss of the surface. The index weights these features rather than treating them as equal (RHI development study).

Neutrophils and surface injury are central markers of active microscopic disease. Chronic architectural changes can remain even when acute inflammation resolves, so the broader pathology narrative still matters. The report should also say where biopsies were taken, because UC activity can vary by segment and tiny samples do not represent every centimeter of colon.

Studies use definitions such as histologic response, remission, or normalization. A commonly used remission concept combines a low RHI with no neutrophils in the epithelium, but the exact endpoint must be read from the study. A total alone can conceal which feature remains active.

RHI versus Nancy and Geboes

Nancy uses five ordinal grades and is relatively quick to apply. Geboes is hierarchical and more granular. RHI produces a weighted continuous score designed and validated for responsiveness in UC trials. They all examine overlapping biology but organize it differently, so their values are not numerically interchangeable (ECCO histology overview; ECCO reporting review).

International consensus work has recommended RHI or Geboes for UC histology outcomes in trials, while recent histopathology guidance emphasizes validated scoring and explicit definitions (CORE-IBD; histopathology consensus). Routine pathology reports may describe activity without calculating RHI, and that can still provide clinically useful information.

The exact remission threshold should always accompany a reported result.

What to compare with the biopsy score

MES and UCEIS grade visible mucosa, while RHI grades microscopic tissue. CRP measures systemic inflammation. Fecal calprotectin reflects intestinal neutrophilic inflammation but is affected by sampling time and other intestinal conditions. Symptoms record the patient experience. Disagreement can be real because each layer asks a different question.

STRIDE-II treats histologic healing in UC as an assessment of depth of remission, not a formal stand-alone treatment target (STRIDE-II). Keep the RHI total, four component grades if available, highest-activity segment, biopsy sites, endoscopic score, medication exposure, symptoms, CRP, and calprotectin together. Do not change treatment from a pathology score alone. RHI is most useful for following microscopic change when the same validated method and adequate segment sampling are used over time.

Questions that make the number more useful

Before comparing this result with an older one, confirm that both records used the same named instrument, version, scoring range, and assessment window. Ask whether the total represents the worst segment, a sum across segments, an average, or a single day. If a clinician or study used a modified version, record exactly what was removed or added. Also check whether treatment, bowel preparation, infection, missed diary entries, or incomplete examination affected the inputs. A small apparent change may reflect measurement conditions rather than a meaningful change in disease.

At a visit, useful questions include: What domain does this score measure? Which component drove the result? What would count as response or remission in this setting? Does the number need confirmation with blood work, stool testing, endoscopy, biopsy, or imaging? Is the goal to follow symptoms, current inflammation, structural damage, treatment response, or quality of life? Asking these questions turns an unfamiliar total into a specific clinical conversation.

For personal tracking, avoid combining unlike scores on one graph. Keep each instrument in its own series and place objective tests on parallel timelines. Save the source report because the component details may matter later. Note both the sample or procedure date and the date you received the result. Trends are easiest to interpret when the method and timing stay consistent. If the number conflicts with how you feel or with another test, bring the disagreement to your care team instead of choosing the result that seems most reassuring. Discordance is often the reason to look more closely.
