Starting a new medication for inflammatory bowel disease means moving through two distinct phases with different rules. Induction is the loading period, when higher or more frequent doses are used to bring inflammation down. Maintenance is the steady schedule that follows, meant to keep the disease quiet for years. Care teams use these words constantly, but patients are rarely told that what they should be watching also changes between the two stages. Knowing what to track during IBD treatment at each phase makes appointments more productive and gives a gastroenterologist real data instead of a vague recollection of the past few months. This guide breaks the monitoring down stage by stage.
Induction and Maintenance Describe Two Different Jobs
The split between the two phases is written directly into the FDA prescribing information for most IBD drugs. Infliximab is given at 5 mg/kg intravenously at weeks 0, 2, and 6, followed by 5 mg/kg every 8 weeks for both Crohn's disease and ulcerative colitis. Adalimumab uses 160 mg on day 1, 80 mg on day 15, and then 40 mg every other week starting on day 29. Vedolizumab is dosed at 300 mg intravenously at weeks 0, 2, and 6 and then every 8 weeks, or 108 mg subcutaneously every 2 weeks beginning at week 6. Ustekinumab uses a single weight-based intravenous dose followed by 90 mg subcutaneously 8 weeks later and every 8 weeks after that. Induction is compressed and front-loaded. Maintenance is spaced out and repetitive. The tracking should mirror that shape.
What to Track During Induction
Induction is the noisy stage, so the log should be dense. Daily stool frequency, presence and amount of blood, urgency, night-time waking, and abdominal pain give the clearest picture of early response, because the international STRIDE-II consensus names symptomatic relief and normalization of serum and fecal markers as the short-term treatment targets. Record every infusion or injection date, since the interval between induction doses is short and a shifted appointment changes the whole sequence. Note infusion reactions, injection site reactions, fevers, and any new infection, because these are the events a nurse will ask about before the next dose is released.
Corticosteroids often overlap with the first weeks of a biologic, and the taper is worth tracking on its own line. The National Institute of Diabetes and Digestive and Kidney Diseases notes that corticosteroids for Crohn's disease should only be used short term. A log showing that symptoms crept back at 15 mg of prednisone tells a clinician something a symptom diary without dose context cannot.
The Week 14 Decision Point
Induction has a deadline, which is why the early record matters so much. The vedolizumab label instructs prescribers to discontinue the drug in patients who do not show evidence of therapeutic benefit by week 14. Other agents have similar assessment windows. When that conversation happens, the question is whether the drug is working, and the answer depends on what was captured in the preceding three months.
Baseline numbers make the comparison possible. A fecal calprotectin and C-reactive protein drawn before the first dose, then repeated during induction, turn a subjective sense of improvement into a measurable trend. The 2025 American College of Gastroenterology ulcerative colitis guideline emphasizes regular monitoring when initiating a new treatment and the use of fecal calprotectin to assess response, with endoscopic improvement as the goal that predicts sustained steroid-free remission.
What to Track During Maintenance
Maintenance tracking is lighter but should never stop. The American Gastroenterological Association suggests using fecal calprotectin under 150 micrograms per gram, normal fecal lactoferrin, or a normal C-reactive protein to rule out active inflammation in patients with ulcerative colitis who feel well. The companion Crohn's disease guidance uses the same calprotectin threshold of 150 micrograms per gram and a C-reactive protein under 5 mg/L, with a stricter cutoff of 50 micrograms per gram after surgery. Both documents recommend combining biomarkers with symptoms rather than relying on symptoms alone.
There is trial evidence behind that approach. The CALM study randomized 244 patients with Crohn's disease to tight control guided by C-reactive protein and fecal calprotectin or to symptom-driven management, and mucosal healing at 48 weeks reached 46 percent with tight control versus 30 percent with clinical management. A maintenance log that pairs occasional lab values with a short symptom entry is the patient-side version of that strategy.
Dose Timing, Delays, and Missed Appointments
The single most useful thing to record during maintenance is when each dose actually happened. Adherence falls sharply once the loading phase ends. In a Canadian cohort of 215 patients on infliximab, 77.9 percent were adherent to induction infusions but only 31.6 percent stayed adherent to the scheduled maintenance regimen, and 92.1 percent had at least one delayed maintenance infusion. Feeling better is a common reason people stop. In one cohort, 42.7 percent of intentional nonadherence was attributed to stopping treatment after symptoms resolved.
Delays and gaps matter clinically because they affect drug levels. AGA guidance suggests reactive therapeutic drug monitoring to guide treatment changes in adults with active IBD on anti-tumor necrosis factor therapy. Interpreting a trough level requires knowing exactly when the last dose was given, so a dated dose record is what makes the test meaningful.
Carrying One Record Across Both Stages
The value of stage-specific tracking compounds when the two logs live in the same place. A record that begins with pre-treatment baseline symptoms and labs, tightens during induction, and then thins into a steady maintenance rhythm shows the full arc of a therapy rather than a snapshot. When response fades after two years, the question becomes whether symptoms returned gradually or abruptly, whether doses drifted late, and whether calprotectin rose before the symptoms did. Those answers exist only if someone wrote them down. The tracking effort scales with the stage, heavy at the start and light later, but continuity across both is what turns a personal log into clinical evidence.
