Many people with inflammatory bowel disease notice the same thing every cycle. The first weeks after an injection or infusion feel steady, and then, somewhere in the final stretch before the next dose, urgency creeps back, stools loosen, joints ache, and fatigue returns. By the time the next dose arrives, the symptoms settle again. Patients often describe this as the biologic wearing off early, and it has a plausible pharmacological basis. It also has several other possible explanations, which is exactly why the pattern needs to be documented rather than described from memory at a rushed appointment.

A single rough week before a dose is an anecdote. The same symptoms appearing at the same point in three or four consecutive dose cycles is data. This guide covers what to track, how to record it so the timing is unambiguous, and what your gastroenterologist can do with it.

Why symptoms return before the next dose

Biologic drug concentrations in the blood are not flat. They peak after each dose and fall to their lowest point, the trough, immediately before the next one. Adalimumab has a mean terminal half-life of roughly two weeks, ranging from 10 to 20 days, and standard maintenance dosing in Crohn's disease and ulcerative colitis is 40 mg every other week, according to the HUMIRA prescribing information. Infliximab maintenance is 5 mg/kg every 8 weeks per the REMICADE label. Vedolizumab has a serum half-life of about 26 days with intravenous maintenance every 8 weeks or subcutaneous dosing every 2 weeks, per the ENTYVIO label, and ustekinumab has an estimated median terminal half-life of about 19 days in IBD populations with 90 mg subcutaneously every 8 weeks, per the STELARA label.

If your body clears the drug faster than average, or if you have developed antibodies against it, the trough may fall below the level needed to suppress inflammation. Antibodies to adalimumab are associated with reduced serum drug concentrations, and infliximab antibody-positive patients have higher clearance and reduced efficacy, as both labels state. Secondary loss of response is common: the annual risk has been estimated at roughly 13% per patient-year for infliximab and 20.3% for adalimumab, with 23% to 46% of patients eventually needing dose intensification. A large cohort study found loss of response rates of 17.2% per patient-year in year one, falling to 4.8% by year four.

What to track across dose cycles

The single most important variable is the number of days since your last dose. Recording symptoms by calendar date alone makes the pattern nearly invisible, because dose dates shift with holidays, pharmacy delays, and infusion scheduling. Recording each symptom entry as "day 9 of 14" or "week 6 of 8" makes it obvious.

For each day, log stool frequency, presence of blood, urgency, abdominal pain on a consistent 0 to 10 scale, nighttime awakening, and fatigue. Add extraintestinal symptoms such as joint pain, mouth ulcers, or skin lesions, since these often follow the same trough timing. Note anything that could confound the picture: a course of antibiotics, an NSAID for a headache, a stomach bug in the household, a stressful stretch at work, or a change in diet.

Track dose administration separately and precisely. Record the actual date each dose was given, not the date it was scheduled. Note any missed, delayed, or self-deferred doses, injection site problems, and whether refrigerated pens were stored properly. A pattern of drifting injection dates can produce trough symptoms without any true loss of drug efficacy, and your care team needs to be able to distinguish the two.

Pair symptoms with objective results

Symptoms alone are not enough to justify changing therapy. The AGA recommends a monitoring strategy that combines biomarkers and symptoms rather than symptoms alone, both in ulcerative colitis and in Crohn's disease. A fecal calprotectin below 150 μg/g is treated as normal in both guidance documents, with a CRP under 5 mg/L as the corresponding serum threshold in Crohn's disease. If you can, arrange a calprotectin sample during your worst pre-dose days rather than in the calm week after a dose, and log the collection date against the dose cycle like everything else.

Before escalating a biologic, clinicians are advised to confirm active inflammation and rule out non-inflammatory explanations including gastrointestinal infection, bile acid malabsorption after ileal resection, small intestinal bacterial overgrowth, strictures, and overlapping irritable bowel syndrome, as reviewed in this analysis of loss of response management. Drug levels and what your team can do

If the pattern holds up, the next step is usually therapeutic drug monitoring. The 2025 ACG ulcerative colitis guidelines recommend measuring serum drug levels and antidrug antibodies in patients losing response to an anti-TNF, and recommend fecal calprotectin for evaluating suspected relapse during maintenance.

Low drug levels without meaningful antibodies generally point toward dose escalation or interval shortening. The infliximab label notes that some adults who initially respond may benefit from increasing to 10 mg/kg every 8 weeks, and one study found shortening the infliximab interval from 8 to 6 weeks was at least as effective as doubling the dose. The TAXIT trial in Gastroenterology randomized 263 patients and found concentration-based dosing targeting 3 to 7 μg/mL produced fewer flares than clinically based dosing, at 7% versus 17%. High drug levels with persistent symptoms and no antibodies suggest the inflammation is being driven through a pathway this drug does not reach, which usually means switching class rather than adding dose.

Bring the log as a compact summary: dose dates, symptom scores plotted against days since dose, biomarker results, and confounders. Three or four annotated cycles give a gastroenterologist something concrete to act on, and they turn a vague complaint about the medication fading into a specific, testable question about trough exposure.
